Hospice care is:
- Patient-centered, physician-directed and nurse-managed
- Focused on comfort, best possible quality of life, and coordination of care
- A medical model of care that meets the physical, emotional and spiritual needs of those living with life-limiting illness
- An interdisciplinary approach that includes a team of physicians, nurses, social workers, spiritual counselors, volunteers and therapists as appropriate
- Offered in patients’ homes, long-term care facilities (nursing homes), hospitals or hospice inpatient units
